0402台铁408次太鲁阁号事故是2021年4月2日上午9时28分45秒发生在台湾花莲县秀林乡的台铁北回线和仁段清水隧道北口的列车脱轨事故。载有498名乘客的太鲁阁自强号（太鲁阁号）在行经该隧道时，与滑落边坡侵入路线的工程车碰撞出轨后冲入隧道中且擦撞隧道壁，并造成多名旅客遭抛离原位，造成49人死亡和247人轻重伤。本次事故是台湾灾难史上死亡人数仅次于1948年新店溪桥火烧车事故的铁道事故，死亡数超过2018年普悠玛列车脱轨事故。
台铁紧急动员抢修队总共105人，并出动2列机务抢修列车，2部25吨吊车、挖掘机2部，预计事故车辆拖离要5天、轨道修复1天，电车线及导电轨修复1天，最终耗时7天抢修完成。

在花莲列车事故发生后，“76行者遗体修复团队”第一天出动58名修复师，赶赴现场义务协助家属修复遗体。召集人陈修将谈而这次事故罹难者，大体受损情形较两年前的宜兰普悠玛事故更加严重，许多家属看到亲人残破躯体，几乎个个情绪崩溃、放声大哭，就算已看尽生死的殡葬业者，内心也难免激动。

陈修将印象最深刻的亡者，莫过于现年才4岁的陈小妹妹，他哽咽地说，自己也有个4岁女儿，深知做父母的看到孩子变成那样，内心会有多痛、多煎熬” 这次51位亡者中，当属陈小妹的受损最严重，她的伤口遍布全身，还有多处缺损，修复难度最高，会尽可能将陈小妹修复完整、进行填充，重新缝合定位后再梳洗穿衣，希望以最完美的状态交还给其父母。
